Reponses From Hospitals
The following statements were submitted by hospitals in reponse to CBS 2 investigative reporter David Goldstein's report: "Calif. Hospitals Report Serious Medical Mistakes" St. Joseph Hospital, OrangeSt. Joseph Hospital deeply regrets the incident that occurred in February 2008. Our hearts and prayers continue to be with the patient, family, hospital staff and physicians.
St. Joseph Hospital takes patient care very seriously. As such, the hospital has procedures in place designed to ensure the delivery of high-quality care. These procedures include auditors in the operating room, surgical site markings by the surgeon and Crew training, which is similar to the training flight crews undergo to ensure safety in the cockpit. These procedures ensure high-quality care while fostering a culture of accountability and empowerment for all surgical team members in the operating room.
St. Joseph Hospital remains steadfast in its commitment to caring for patients in an environment that fosters clinical excellence, continuous process improvement and compassionate care.
Olive View-UCLA Medical Center, SylmarOlive View-UCLA Medical Center takes reporting requirements very seriously, cooperates fully with investigations, and takes immediate corrective actions to address findings when they are issued. As a public institution, we are a committed to reporting all incidents as required by law.
The incidents contained in the reports obtained by KCBS/KCAL refer primarily to administrative rather than patient care issues, but in each case, the hospital implemented immediate corrective actions that were approved by the State.

This case is currently in litigation. As such, we are not at liberty to respond. However, we do believe that we have met all the requirements under the law with regard to the reporting of this case to the state.

Any incidence regarding patient safety are assessed rapidly and corrective action is taken immediately. Programs and technologies designed to provide the safest evidence-based practices for our patients continue to be implemented and are reviewed regularly as part of our process of continuous patient safety improvement.
San Antonio Community Hospital, UplandPlease let me clarify that the document you sent to us was a CMS Validation Survey. During the survey a team reviews all areas of the hospital for possible deficiencies. The document identifies potential areas of interest. The hospital completed a corrective plan of action and was resurveyed based on the submitted plan. The CMS survey committee gave the hospital an all clear report with no deficiencies or further follow up.
San Antonio Community Hospital takes reporting of adverse events seriously. The definitions of a reportable event can be interpreted in different ways. Because of this we are more likely to report an event then some other hospitals. Transparency in reporting events is an important part of making healthcare better. San Antonio believes in the intent of the legislation to improve the quality of care in hospitals.
